How to order a scan report?

Step 1

Choose the type of scan (CT – computed tomography / MRI – magnetic resonance imaging) and the anatomical area being examined. Add the selected report to your basket.

Step 2

If everything in your basket is correct, proceed to payment. Fill out the form with your first name, last name, phone number, email address, date of birth, and the name of the scan.

Step 3

Note: Please tell us about your symptoms and briefly describe your medical history. To proceed, accept the terms and conditions, and confirm that you are acting on your own behalf. If you are acting on behalf of someone else, please complete the relevant form.

Step 4

Make the payment for your order.

Step 5

Check your email. You will receive further instructions on how to send your scan images.

Step 6

When your report is ready, you will receive a text message from us.

1. The patient chooses to send the file without logging in.
2. The patient accepts the terms and conditions.
3. The patient enters opisy@eurodiagnosis.pl in the “Email to” field.
4. The patient enters their own email address in the “Your email” field.
5. The patient attaches the scan images in DICOM format.
6. The patient may also attach medical documentation such as referrals, previous reports, blood test results, etc.
7. The patient receives an email confirmation that the file was sent to opisy@eurodiagnosis.pl.

We reserve the right to refuse the service in the following cases:

  • The patient fails to provide complete information required to carry out the service.
  • The scan images (CT/MRI) provided are non-diagnostic or older than 3 months.
  • The scans of previous diagnostic reports are illegible.
  • There is a justified suspicion that the patient intends to use the service in a manner inconsistent with its intended purpose or in breach of applicable laws.
  • The service user violates the Terms and Conditions, applicable laws, or generally accepted social or moral standards.

In the above cases, our coordinator will contact you to explain the situation. A refund for the scan report will be issued within 14 days of this contact.

The reporting time is approximate and may vary depending on the diagnosis, the complexity of the case, as well as the quality and type of the submitted scans.
